    I don't know who made the warnings. It just states what they were for and when. I only know if they told me.

    I also agree that the warning levels should be available to the user that commits them. I also think the offense should have a permalink to the offending post.

    And no. It just states what for (the reason is written by the moderator. There is no defined list to pick from.) and when.
