There was moisture in the ballast tanks, that froze inside the system, preventing blowing the tanks. They also lost propulsion (unable to perform a powered ascent) when the reactor scrammed. They were unable to restart the reactor or using existing steam due to procedures restricting such things. While we dont know EXACTLY what happened on the USS Thresher, we do know many ways it could have happened, and then worked to protect against each possiblility to prevent such a tragedy in the future.
