Part-6 of serious disasters. A piece of foam hits shuttle columbia during launch. Not seen by CG or crew that it punched a hole in the wing of the shuttle, and damaged the heat protection.

I remember that day. I've seen it live in TV from the MCC. MCC tried to find out what the f*** happens, in between the voice of the capcom "Columbia, Houston UHF check, com check." That was so surreal. -
