Part-6 of serious disasters. A piece of foam hits shuttle columbia during launch. Not seen by CG or crew that it punched a hole in the wing of the shuttle, and damaged the heat protection.

that's why mission control should expect that damage in the wing and make a rescue mission to safely bring the astronauts back to the earth with that rescue ship.

Engineers ordered imagery through telescopes, but were discouraged by mission control, despite a simulation predicting serious damage. Only later in the investigation were a few critical mistakes in the protocol revealed, as nobody had expected the thermal protection tiles to be so brittle, and mission control overestimated their thickness. All in all, it /was/ noticed and there was a big deal of discussion about it.

The piece of foam was noticed by mission control through a camera on the ET. They discussed the issue and figured that it made no difference whether they do something or not, as Columbia had to bring the astronauts back. They also decided not to tell the crew, since they would be nervous and could make a mistake then.
